First visit forms

During my first visit I will need you to complete a Patient Consent form and Medical History form. These can either be emailed to you to complete online or paper copies can be completed on the day of your appointment. I will also conduct a Neurovascular Assessment to check the overall health of your feet and highlight any potential issues there may be. 

Patient Confidentiality/ Data handling

Keeping information confidential is paramount. All personal data provided to me is kept secured and password protected. Cliniko software, which is used to organise and store patient records, is password protected and encrypted with a 2 level secure log in system. Any paper copies of completed forms are destroyed once they have been uploaded to the system. If there is any change to your medical status, please inform me. Your information will be held for up to 7 years after your last appointment, (or 25th birthday if the last appointment was attended while under the age of 18), before the form is destroyed. Your information may be shared with other practitioners in the event of referrals or if I require extra information and/or opinions about the conditions and treatments, but only if your consent is given. 

Medical information

Due to sharp instruments being used in practice, there is a possibility of accidents and injuries occurring. In the event of a wound, I will clean and dress the injury with the appropriate equipment and provide you with information on wound management. Once this is completed the responsibility of wound management will move to you. Ineffective wound care or negligence may result in serious medical consequences, including infections.

Consent to images

I may take before and after photographs of the area of interest for comparison within your medical notes, to display online, (website or social media), or for advertising purposes. If you consent to having your pictures shared online, personal information, (including name and date of birth), will never be included in any form and no personal characteristics will be displayed, such as tattoos or birthmarks. Any description will be limited to the medical issues discussed and the chosen treatment plan. (Example: “A gentleman / lady came in with …… . We decided to treat it with…. and here are the results.”)
